Registration Form 2015/16 Season
A non-refundable $25 Registration Fee is due at the time of enrollment for all students, no matter which payment option is chosen. Please note that those choosing the monthly payment option (Option 1), first and last month’s tuition is also due at this time. A late fee of $25.00 will be charged to your account for each payment made after the 15th of the month for those choosing Payment Option 1.
EEDS accepts MC/Visa/AMEX and Discover credit cards, checks or cash. Returned checks will be charged a $35.00 fee. Payment may be made online or at the studio kiosk.
All Students must register in person at Essential Elements Dance Studio for the 2015-16 season.
We look forward to meeting your family and guiding you through a tour of our new facility on Bethany Road in Hazlet, NJ. We can discuss our classes and schedule available to your young dancer at this time.
Upon registration, you will receive an email confirmation of class placement and your dancer's weekly schedule. Additionally, you will review your tuition option choice made upon registration.
At this time, you will have the Teacher Identification Code for On Line Dancewear purchases.
Classes begin on Tuesday, September 8th, 2015.
After initial payment and Registration Fee of $25.00 per dancer upon studio visitation, you will make all payments on line through Essential Elements Dance Studio Website Secure Portal or our SquareUp Market Place..
September & June tuition installments and a $25.00 registration fee are due upon enrollment.
This will help us complete your child’s placement in a timely manner. Please choose your payment option on the EEDS website. EEDS accepts MC/Visa/AMEX or Discover credit cards, checks or cash. Returned checks will be charged a $35.00 fee. Payment may be made online or at the studio kiosk.
A late fee of $20.00 will be charged to your account for each payment made after the 15th of the month for the monthly payment option.
*No exceptions will be made for the Tuition policy.
PLEASE NOTE: If you choose to cancel your enrollment, there are no refunds, We do not adjust tuition rates.
Students absent due to illness, inclement weather or holiday will be permitted to make up their missed class. Our office staff is available during class
time. Protocol: All Students must follow the Dress and Hair Code found on the website. Please be on time, respect your teachers and your fellow students. Please call the studio to inform of student absence or if you’re late picking up. Enjoy your class, have fun and do your best!!
